Reliability and validity of the power-mobility community driving assessment.

The power wheelchairs Community Driving Assessment (PCDA) is a performance-based measure designed to assess driving performance of individuals using power wheelchairs or scooters in community environments. This article reports the results of pilot testing and an evaluation of the assessment’s reliability and validity. Pilot testing was conducted with a random selection of Canadian occupational therapists working in the area of mobility. Although the response rate was very low, feedback confirmed the utility of the measure and contributed to one substantive scoring revision. Reliability and validity testing was conducted with a sample of 34 drivers. Internal consistency results were positive. Interrater reliability was fair to high but limited by the lack of variability in the scores. Construct validity hypotheses were tested on the relationships between PCDA scores and vision, perception, cognition, and environmental accessibility. Results indicated no relationships between the PCDA and perceptual and cognitive function and only a weak trend for a relationship with environmental accessibility. Concurrent validity was established: PCDA scores were positively associated with the judgments of therapists familiar with the driving performance of participants. In summary, the PCDA has moderate to good reliability, and content and concurrent validity results were found. More research is needed, particularly on the underlying constructs of successful driving performance. At this point, rehabilitation professionals and their clients are urged to use this assessment to establish driving performance rather than relying on assessments of perception, cognition, or environmental accessibility to predetermine whether someone will receive power mobility. Clinicians may find this a useful tool to identify where clients are able to drive safely in community settings, to identify specific learning needs, and, through those, to promote independent living for drivers of power-mobility devices

In building your Network Marketing business, it is critical to know exactly who your target market is. Clearly defining your target market with upfront research should always be your first step in planning for your Network Marketing business.

These are several key questions to ask yourself when doing your research.

Here’s a short list that should get you going:

* What is the demographic of your customers? e.g. income level, gender, education, age, marital status * Do they have a specific location? e.g. country, state, city, county * What do they want? * What are their key problems? * What do they currently buy?

Once you clearly define who your potential customers are, you can begin to set up your marketing strategies around them. But until you do, you should not even consider starting your Network Marketing business (or any kind of business).

People so often come up with their product which, in their eyes, are superior in quality, and then start looking for customers who can benefit from the use of this product. When people take this approach, they are at the risk of investing a lot upfront to come up with the products to sell while not really knowing whether they will sell.

While searching for the customers, they will need to store their inventories so that they can ship the products quickly as soon as they find the customers. This causes them to incur unnecessary expenses for storage and possibly lose their initial investment if they can’t find the customers to buy them.

If you can first spot a group of people who has a specific want, need, or problem they want a solution for, and then come up with the exact things they are looking for, it makes things so much easier with much less risk.

Here are a few examples:

* Starbucks ==> coffee drinkers * Borders ==> book lovers * Eye doctors ==> people with poor vision

Each of the above has a specific group of people who has specific wants, needs and problems they want a solution for. It wouldn’t have been wise for Starbucks to come up with the best tasting coffee without knowing whether there’s enough people who can and will spend $3 bucks for a cup of joe.

Now, when it comes to target markets for your Network Marketing home business, there are several possible categories depending on your approach. Here’s a list of possible target markets:

* People who have purchased a network marketing business opportunity before (or who are already in network marketing, who are currently spending money on pursuing a home based network marketing business), who already recognize their need for a network marketing business.

* People who are presently researching your Network Marketing company and seeking the best sponsor, team and upline on Google, Yahoo, MSN or even offline talking to people. These people have heard of the Network Marketing company, products, marketing system, and/or business opportunity, and are doing their “homework” to validate the information about the company, products, marketing system, and/or business opportunity and the best team to join.

* People interested in finding a way to make money, but who aren’t specifically aware that they need a Network Marketing business (yet). These people are looking in your direction with obvious interests, but are still in the “tire-kicking” stage. Until they actually spend money by starting a home based business, or invest money in learning resources (books, CDs, DVDs, seminars), they are just looking.

* People who already buy or use a related product or service of your Network Marketing company.

* People looking on the web for your specific product by name. These people are the hottest product leads.

Once you grasp which target market you want to attract to you, you simply need to start giving away valuable content that they are looking for through various point-of-contacts e.g. social networking sites, social bookmarking sites, etc.

Remember, you don’t want to chase after your target market. You want to always ATTRACT them to YOU. Give to get. Give without want. And you’ll soon be attracting them to YOU and your Network Marketing business.

3 Benefits Of Continous Learning

We are always taking in new information but the question is WHAT are we learning? As human beings we are always growing, but we need to ask ourselves “in what direction”? Are we choosing the direction that we want to go?
Many people are committed to their “stories” which really are excuses for not taking action. They live their life by them. I used to be one of the people committed to my stories/excuses. Believe me, it was learned behavior. I learned it from many different sources such as family, school, friends and work environments. For example, because of the influence of others at work, I always believed that it was best for me to stay home 4 work nights each week and only be out late on a work night once a week. As a result there are many things I have not done because of that limiting belief that it was “not good for me to go out too many times during the week”. One of the things I missed out on was learning Spanish, something I have wanted to do for years.
So why is it that we should be making conscious choices about which information we take in so that we are Continuously Learning?
3 Benefits of Continuous Learning:
1) Protect your future for you and your family…
We can protect ourselves by learning new skills. With these new skills we can protect ourselves from being laid off or downsized. For example, if you were to go to school once a week and learn something that makes you a more valuable commodity in the marketplace or learned a new skill in a different arena, you could be prepared for downsizing or layoff when it happens. We know in this changing world that there is no escape from change, it surrounds us daily and the pace of change is accelerating.
Are you sitting around waiting for change or are you directing the change that happens to you? We know that we do not get a promotion without a test. Have you ever applied for a job that you really wanted? Did you study, do research on the company and their visions and goals. Prepare yourself for that interview/test? If you really wanted the job, then I am sure you did. Learning something new, preparing for that business opportunity protects us and allows us to get out of our “comfort zone”. Nothing ever happens in the comfort zone, we stay right where we are, even though the world is changing around us. Do you want to get left behind?
2) Learning can change your life
I have just completed a learning event with Live Out Loud and Loral Langemeier in Lake Tahoe. Believe me, it was a stretch and really assisted me in getting outside of my comfort zone. My life has changed for the better because of that experience and I am preparing myself for the future!
The events that have happened in my life where I have had to get out of my comfort zone have all in some way, big or small, improved my life. There are still times where I want to wallow in my comfort zone. However, now I know the perils of staying there, so I do not stay there very long.
When I first started on my journey of building my home business, our dream was to be able to sponsor 10 children with “The Plan”. As we have learned and grown through the process our focus is now to sponsor 100 children. The learning has changed our attitude about life and money, which of course has resulted in changing our lives. The focus of our life has changed for the better.
3) Learning can keep you young
Learning assists us in adapting to change. Think of a time when you dreaded having to learn something new. Why were you dreading it? Was it because you are not in the habit of learning? Did you believe it would be too hard? Did you not see any benefit for the learning? Getting back into the habit of learning is just like starting out on a bicycle, it takes a lot more effort to get going than it does once you have attained the desired speed. When we are learning we are involved with life, we are out with our peers or a whole new group of people.
What are new information are you feeding yourself? I realized that the fiction books I was feeding myself were simply a form of escape. Very similar to the young boy we met on the streets of Le Ciba in Honduras. He was one of the kids who were abandoned to the streets as young as the age of two. What he did to escape the reality of his own life was to sniff gasoline. After meeting him I realized that I was doing the same thing by reading too many fiction books.
After that I started feeding myself with more positive material. If this is one of your escape mechanisms and you desire to change it, you may want to switch the type of books that you read. Biographies are a great way to switch from fiction because they are stories too.
Once we have begun to change our learning habits by making conscious decisions about the information we feed ourselves on, the next step is to teach what we have learned. Teaching others is the best way to learn it even better. One of my great desires is for my family and friends to learn how not to get left behind. That is the reason I choose to write these articles, because assisting you in your learning also assists me in understanding better myself.

Technological advances in powered wheelchairs

During the last 40 years, there have been revolutionary advances in power wheelchairs. These unique wheelchair systems and power wheelchairs, designed for the physically immobile patient, have become extremely diversified, allowing the user to achieve different positions, including tilt, recline, and, more recently, passive standing. Because of this wide diversity of powered wheelchair products, there is a growing realization of the need for certification of wheeled mobility suppliers. Legislation in Tennessee (Consumer Protection Act for Wheeled Mobility) passed in 2003 will ensure that wheeled mobility suppliers must have Assistive Technology Supplier certification and maintain their continuing education credits when fitting individuals in wheelchairs for long-term use. Fifteen other legislative efforts are currently underway in general assemblies throughout the US. Manufacturers, dealers, hospitals, and legislators are working toward the ultimate goal of passing federal legislation delineating the certification process of wheeled mobility suppliers. The most recent advance in the design of powered wheelchairs is the development of passive standing positions. The beneficial effects of passive standing have been documented by comprehensive scientific studies. These benefits include reduction of seating pressure, decreased bone demineralization, increased bladder pressure, enhanced orthostatic circulatory regulation, reduction in muscular tone, decrease in upper extremity muscle stress, and enhanced functional status in general. In February 2003, Permobil, Inc., introduced the powered Permobil Chairman 2K Stander wheelchair, which can tilt, recline, and stand. Other companies are now manufacturing powered wheelchairs that can achieve a passive standing position. These wheelchairs include the Chief SR Powerchair, VERTRAN, and LifeStand Compact. Another new addition to the wheelchair industry is the iBOT, which can elevate the user to reach cupboards and climb stairs but has no passive standing capabilities. In addition, the physically immobile patient must be seated on an ERGODYNAMIC Seating System 2000, which is inflated by the alternating pressure compressor 8080. This seating system has a deep center seam between the two ischial-support chambers, which provides a recess for the coccyx. The pre-ischial crossbar compartment inflates during each cycle to prevent the pelvis from slipping forward. It is essential that the physician of the immobile patient order two ERGODYNAMIC Seating Systems 2000 because the patient must have an additional seating system in the case one leaks. Moreover, two compressors are necessary because each compressor must be serviced after 2500 hours of use. For the protection of the consumer, these pressure relief systems must be supplied and serviced by a Certified Rehabilitation Technology Supplier such as Wheelchair Works Inc. Despite the indisputable scientific evidence of the medical benefits of passive standing for the immobile user, few individuals have access to these revolutionary wheelchairs. Consequently, it is mandatory that the medical community, headed by specialists in physical and occupational therapy as well as rehabilitation medicine, CRTS, and manufacturers collaborate in a national education campaign to convince Medicare/Medicaid and all commercial insurance companies to approve immediately these assisted technologies. This program is essential so that the physically immobilized patient can achieve the undisputed physical benefits of passive standing.

This interim final rule conforms our regulations to section 302(a)(2)(E)(iv) of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003 (Pub. L. 108-173). This rule defines the term power mobility devices (PMDs) as power wheelchairs and
medicare power wheelchairs. It sets forth revised conditions for Medicare payment of PMDs and defines who may prescribe PMDs. This rule also requires a face-to-face examination of the beneficiary by the physician or treating practitioner and a PMD prescription and pertinent parts of the medical record that the durable medical equipment supplier maintains in records and makes available to CMS or its agents upon request. Finally, this rule discusses CMS’ policy on documentation that may be requested by CMS or its agents to support a Medicare claim for payment, as well as the elimination for the Certificate of Medical Necessity for PMDs.

BACKGROUND: To evaluate the effect of a 7-week low-intensity hand rim power wheelchairs training on the submaximal metabolic cost, mechanical efficiency and propulsion technique in able-bodied participants. METHODS: Participants were randomly divided over an experimental group (n=14) and a control group (n=7). The experimental group received 7 weeks wheelchair training (3 week(-1), 70 min) at a low intensity (30% of the heart rate reserve), whereas the control group did not receive training. During pre- and post-tests, submaximal exercise was performed on a stationary wheelchair ergometer at fixed levels of power output. Mechanical efficiency, oxygen uptake, heart rate, timing parameters and stroke angles were measured. Video recordings were made to determine the stroke pattern. FINDINGS: Mechanical efficiency increased and metabolic cost decreased significantly in the experimental group compared to the control group. Push time increased and cycle frequency decreased as a result of training. The stroke angle increased in the experimental group during the training period. The experimental group preferred double-looping over propulsion, while the control group mainly used single-looping over propulsion patterns during the post-test. INTERPRETATION: A low-intensity, 7-week training protocol has a beneficial effect on the mechanical efficiency and metabolic cost of power wheelchairs propulsion in able-bodied participants. The improved mechanical efficiency seems to be the result of changes in propulsion technique that were found

OBJECTIVES: To determine if a standard of care for wheelchair provision exists within the participating centers and if there is disparity in wheelchair customizability among the study sample. DESIGN: Convenience sample survey. SETTING: Thirteen Model Spinal Cord Injury Systems that provide comprehensive rehabilitation for people with traumatic spinal cord injury (SCI) and that are part of the national database funded through the US Department of Education. PARTICIPANTS: A total of 412 people with SCI who use wheelchairs over 40 hours a week. INTERVENTION: Survey information was obtained from subjects via telephone and in-person interviews and from the national database. Collected information included age, race, education, level of injury, and wheelchair funding source. MAIN OUTCOME MEASURES: Number and type (manual or power) of wheelchairs. Wheelchair customizability as defined by design features (eg, adjustable axle position, programmable controls). RESULTS: Ninety-seven percent of manual wheelchair users and 54% of power wheelchairs users had customizable wheelchairs. No power wheelchair user received a wheelchair without programmable controls. Minorities with low socioeconomic backgrounds (low income, Medicaid/Medicare recipients, less educated) were more likely to have standard manual and standard programmable power wheelchairs. Older subjects were also more likely to have standard programmable power wheelchairs and power wheelchairs. CONCLUSIONS: The standard of care for manual wheelchair users with SCI is a lightweight and customizable wheelchair. The standard of care for power wheelchairs users has programmable controls. Unfortunately, socioeconomically disadvantaged people were less likely to receive customizable wheelchairs.

Individuals with multiple sclerosis (MS) report decreased satisfaction with their power wheelchairs devices compared with individuals with spinal cord injuries (SCIs). This study (1) investigated the demographic differences between veterans with MS (V-MS) and veterans with SCI (V-SCI) who were issued a wheelchair by the Veterans Health Administration (VHA) and (2) described differences in mobility device prescription. We merged two VHA databases to obtain demographic and wheelchair information for all V-MS and V-SCI in 2000 and 2001. Descriptive information for issued wheelchairs was available for 2,154 V-MS and V-SCI. We found that V-MS were significantly less likely to receive higher quality wheelchairs (manual or power) compared with V-SCI (p < 0.001). The disparity in VHA power wheelchairs prescription between these two groups indicates a need for further research regarding the assistive device prescription process in these populations.

OUTCOMES: A mid-wheel-drive chair afforded improved performance on Furumasu’s tasks compared with a rear-wheel-drive chair. SUMMARY: This case describes the clinician’s role in prescribing power wheelchairs to affect the user’s functional skills, as well as how, in the absence of evidence, clinical experience and patients’ needs can guide clinical decision-making.
